When filling out your CIPP forms, use the following information.
Men
Club Name: Cincinnati Kelts RFC
Club ID#: CC0160
Women
Club Name: Cincinnati-Dayton Women's Side
Club ID#: 100424
If required, both Clubs are in the Ohio Local Area Union and the Midwest Territorial Union.
After you receive your card, please bring me the piece of paper that your card was affixed to. Then make a single page photocopy with the following three things on it:
1. Picture ID (e.g. Driver's License)
2. Proof of Citizenship (e.g. Birth Certificate, Passport, Green Card)
3. Proof of Medical Insurace (same card you give the doctor)
These four things are required to compete in the National Tournament.
